Alliance Pediatric Providers provides skilled home nursing up to 24 hours daily, 7 days a week to medically fragile and medically complex newborns, infants, children and adolescents to 21 years of age. We understand the challenges of caring for a medically complex child who requires numerous medical services and products. Our experienced case managers, along with our bedside RNs and LPNs, work closely with parent and physician to ensure a thorough, expertly coordinated plan of care is implemented. We provide full parent education on all clinical services and we are able to provide care in private homes, school, and medical foster homes.
Transitional Care from Hospital to Home
Transitional care from hospital to home plays a very large role in the road to recovery or health stability. Alliance Pediatric Providers facilitates transitional care through nursing provided by specialized health care professionals. We also specialize in the transition of premature, neonatal infants from hospital to a natural home environment.
Specialized Neonatal Care
We offer an array of advanced equipment and sophisticated services that allows us to care for infants with complex or unusually severe conditions once stabilized and discharged from the hospital. Alliance is recognized by area hospitals as providing excellent post-hospital neonatal care. We are highly experienced in transitioning medically complex premature infants from hospital to home. Our special skills and continuity of care provides an invaluable resource for families at home with their fragile infants.
